"Tennis is Life" Story Contest Rules and Information

Prize to be Awarded
1. One monthly prize: One gift box of 10 Tennis Affinity Greeting Cards. The card style given is at sole discretion of Tennis Affinity.
One Grand Prize: Will be determined by popular vote via Tennis Affinity visitors, with final decision at sole discretion of Tennis Affinity. Winner will be awarded an exclusive piece of tennis jewelry, chosen by representative of Tennis Affinity.

Eligibility
1. No purchase of any kind is required to enter.
2. No limit on entries per person .
3. The contest is open to anyone in the United States except employees of Tennis Affinity, their family members, and those excluded
as stated within the Rules for the Drawing.
4. By submitting an entry, winner agrees to the use of his or her name, city and state, and submission for advertising/publicity purposes
without compensation. Winner also agrees to have their story public on the Tennis Affinity website.
5. Winner must agree to sign an Affidavit of Eligibility if required.
6. All entries become the property of Tennis Affinity and the contest judge.
7. Contest is void where otherwise prohibited by law.
How to Enter
1. Fill out contest submission form on Tennis Affinity web site.
2. Entries must be received no later than 11:59 p.m. Pacific Standard Time on the last day of each month.
4. Internet Entries: Visit Tennis Affinity web site, http://www.tennisaffinity.com/contest.html.. Include your first and last name, and e-mail address.
5. Contest begins January 1, 2009, 12:01 a.m. Eastern Standard Time and has two grand prize deadlines ending July 31, 2009 and November 30, 2009 5:00 p.m. Pacific Standard Time.
6. Not responsible for entries not received, late, lost, incomplete, or illegible entries.
7. Entries must be complete to win.
Awarding of Prize
1. One winner will be selected by the contest judge at random or chosen from the collective entries throughout the contest. One gift box of cards will be given for the each month during the contest.

Entries received by the deadline dates as described above. Winner need not be present to win.
2. Decision of the contest judge is final. Contest judge is affiliated with the sponsor.
3. No substitution in prize will be allowed, and prize may not be redeemed for cash or other consideration.
4. Winner will be notified by e-mail. The winner must respond that they accept the prize and send their contact information (mailing address and phone number) within 48 hours of winner notification or may be disqualified or forfit prize.
